Palliative care aims to improve the quality of life of patients with advanced illnesses and their families, although emotional, social, and spiritual aspects are often underserved. The objective was to analyze the evidence on coping strategies in patients with advanced cancer receiving palliative care and their caregivers, and their impact on quality of life and psychological well-being. A systematic review was conducted according to PRISMA 2020 in PubMed, Scopus, and Web of Science (2005–2025). Twenty-two studies were included. Adaptive strategies, such as acceptance, social support, and positive spirituality, were associated with less distress and better quality of life, while avoidant strategies were associated with greater anxiety, depression, and caregiver burden.
